If you can’t get an answer from a federal agency in a timely fashion, or if you feel you have been treated unfairly, my office may be able to help resolve a problem or get you the information you need.
While we cannot guarantee you a favorable outcome, we will do our best to help you receive a fair and timely response to your problem. We will do our best to contact the federal agency on your behalf and will work on your casefile until we have a final response from the agency for your situation or question. Our office will contact you through the email address provided only when there are updates, requests for additional information, or a final response from the federal agency. Please check your spam/junk mail folders periodically. Be advised that requests with federal agencies can take from 180 to 240 days from our initial Congressional request on your behalf.

- Your address, home phone number and cell phone number so that we can obtain any additional information from you that might be necessary
- Copies of related documents or correspondence that you may have from the agency

For IRS Matters:
Once you have used the link above to submit your digital privacy release form, you should expect to send our office:
1) a color copy of your current, state-issued drivers license or state-issued ID for all taxpayers listed on the tax account;
2) Complete copies of the tax return in question with all attachments, including W-2s and/or 1099s. Each tax return must be signed and dated in blue ink by all taxpayers listed on the tax account on the signature page (on a 1040 form, this is usually page 2).
3) Copies of all IRS/U.S. Treasury notices, CP notices, Offers of Compromise, or documents pertaining tot his matter and tax account.
These required documents should be remitted to our District Office at your earliest convenience through the U.S. Postal Service to: MGP – 1053 Officers Row, Vancouver, WA 98661.
FOR USCIS Matters:
Once you have used the link above to submit your digital privacy release form, you should expect to send our office this form, completed in ink, not digitally.
Please Note: The Privacy Act of 1974 (5 U.S.C. § 552a) requires that Members of Congress or their staff have written authorization before they can obtain information about an individual’s case. We must have your signature to proceed with this type of request.
